GS 5 Year Extension Request Form

Purpose:

This forms exists to give students in Graduate School who anticipate not completing their requirements a request for additional time.

Workflow:

First, the form goes to the Student to fill out, followed by their Advisor, then the Graduate Program Director, a Graduate School signing group that does a pre-check, then the Associate Dean and lastly a post-check by the Graduate School signing group.

Overall Process:

  • The Student will begin the web form and will be required to enter their Campus or EMPL ID, following that they will select their Graduate Program to complete this web form.

  • Then they enter the form to fill out the information that pertains to the request, then they are prompted to sign. 

  • The Advisor then will sign the form followed by the Graduate Program Director to sign.

  • It is then given to the Graduate School signing group to complete a pre-check, and passed to the Associate Dean to make their decision on the extension request and sign. 

  • Lastly, it goes to the Graduate School signing group to look over and initial.

  • Once they initial, the form gets saved in ImageNow and the process is complete.
