Undergrad Request for Grad Credit
Workflow:
First, the form goes to the student to fill out, then to however many instructors are specified (min 1, max 4). Next, it goes to the Graduate Program Director, followed by the department chair, the Assistant Dean for Graduate Enrollment Management, and lastly a signing group.
Overall Process:
The student first needs to enter the names and emails for all the instructors they specify (1-4), the Graduate Program Director, Department Chair, and the Assistant Dean for Graduate Enrollment Management on the webform.
Then they enter the form to fill out the information at the top as well as the course information.
The student will need to know their overall GPA, the GPA in their major, the credits required to complete their undergraduate degree, the total undergraduate credits they’ve completed already, the credits they’re currently taking, and the total graduate credits completed.
They will also need to know the course number, course title, course section, and the credits they’re requesting credit for.
Once they finish filling that out, it goes to the instructor(s) they specified to sign next to their course.
After the instructor(s) sign, the form goes to the Graduate Program Director.
Next, it goes to the Department Chair to sign.
Then it goes to the Assistant Dean for Graduate Enrollment Management to sign.
Lastly, it goes to the signing group to look over and initial at the bottom.
Once they initial, the form gets saved in ImageNow and the process is complete.
