Graduate School Application For Enrollment In A Post-Baccalaureate Certificate Program

Purpose:

This form exists for students in UMBC's Graduate School to enroll in a concurrent Post-Baccalaureate Certificate Program.

Workflow:

The form will begin with the Student, who will supply the name and email of the Program Coordinator on the web form.

Overall Process:

• The Student will supply information about themselves, including their mailing address and phone numbers, as well as information about the certificate program they are applying for. The Student will then sign the form and submit it.
• The form will then go to the Program Coordinator, who will also sign the form.
• Finally, the form will go to the Graduate School signing group, who will provide the final information and initials.
