Purpose:
Workflow:
The Graduate Assistant first fills out the form and signs, followed by the Supervisor.
Overall Process:
On the webform, the Graduate Assistant specifies who the Supervisor is by entering their name and email.
When the Graduate Assistant enters the form, they fill out the academic term and year and check the box that applies to them.
After that, they sign and it goes to the Supervisor.
Once the Supervisor signs, the form is complete.
