GS Plan Change Approval

Purpose:

This form allows students to alter their Graduate program whether it is just the track or a completely new program.

Workflow:

First, the form goes to the student to fill out, then their current Graduate Program Director,  then the new Graduate Program Director, and lastly a Graduate School signing group.

Overall Process:

  • The student first needs to enter the names and emails for the Graduate Program Directors. 

  • Then they proceed to the DocuSign form to fill out, there they will need to enter both the current and new program they are(/intend to) enrolled in, plan code, degree sought, track code and the effective semester. 

  • Once they finish filling that out, it goes to the current Graduate Program Director they specified to sign.

  • After the current Graduate Program Director signs, it goes to the new Graduate Program Director to sign.

  • Lastly, it goes to the Graduate School signing group to look over, enter the date processed, to finalize the change of plan and initial at the bottom.

  • Once they initial, the form gets saved in ImageNow and the process is complete.
