Workflow:

The form begins with the Student, who will provide the names and emails of their Graduate Program Director, Advisor, and Co-Advisor, if applicable, on the web form.

Overall Process:

• On the DocuSign form, the Student will provide their Graduate Program and the working title or topic of their dissertation, and initial to certify that they have completed the responsible conduct of research training. Finally, the Student will fill out the Graduate School Record Form and submit the form.
• The form will then go to the provided Advisor, who will sign, then to the Co-Advisor, if one was listed, and they will also sign.
• The form will then move on to the Graduate Program Director, who will also sign the form.
• Finally, the form will move on to the Graduate School signing group, who will provide the final signature on the form.
