About This Form: No Cost Extension

Reasons for Form:

The No Cost Extension Form creates a schedule for the submission of reports and other deliverables, and states a project end-date. If a project will not be completed by the end-date indicated on the agreement, then an extension request must be completed by the Principal Investigator. If an extension is not requested, the University could be in breach of the agreement, which increases the risk of nonpayment or the potential for a lawsuit.

Workflow:

First, the web form link start with Initiator.

Second, the department.

Third, a copy will be sent to all parties once completed.

How to fill out the form:

Initiator will begin the form by accessing the web form which they will fill out. They will then be redirected to the document where they need to sign off that everything that has been input is correct and filled out the required information.

The department will then review the documentation.
