How do I fill out the Certification of Completion of the Clinical Internship form?

First, the student will fill out this form. The form automatically brings in their first and last name as well as their EMPL ID. The student then selects the term and year that they will graduate. Then the form will go to the student’s advisor (as indicated by the student) to check whether the clinical internship has been completed for the student or not applicable. Then the advisor signs the document. After the advisor finishes signing the document, it gets sent to the Director of Clinical training to look over and sign. Once they are done signing, this form gets saved in ImageNow.
