Telework Renewal
What is the Telework Renewal form and why must I fill it out?
The Telework Renewal form is required in order to renew a completed Telework Agreement. If their has been any change to the terms of the original agreement, a new Telework Agreement must be submitted.
How do I fill out the Telework Renewal form?
The Teleworker can initiate the Telework Renewal form or someone can initiate it for them. The initiator must include the Teleworker's Empl ID in addition to the name and email of each individual in the workflow. They must also indicate any overlap between the Supervisor, Department Head, and Division Head roles to ensure that no one needs to sign twice.
On the form, the Teleworker must provide a Begin Date and End Date for the Telework renewal and then sign (Note: the dates can not be more than one year apart).
