In Ultra view mode, when instructors use the Send Reminder functionality for gradable items in the Gradebook, the reminder is delivered as a course message. However, the expected email notification is not sent to recipients.

Broken Images may occur after editing content and re-saving it.

After the initial save, any images or files embedded in the content editor (e.g., in documents or test questions) are correctly moved from the temporary session folder to permanent storage. However, when the content is later edited and additional images or files are added, these new items are not moved and remain in the temporary session folder causing broken links on subsequent saves.

Session Debug Error When Accessing Student Attempts in Ultra

In Ultra view mode, when a Fill in Multiple Blanks (FIMB) question contains invalid or incomplete answer data (e.g., an empty object in the answers array), attempting to access a student’s submission triggers a runtime error in the grading interface. This scenario may occur due to data inconsistencies introduced during content migration, course duplication, or backend data modifications.

The issue is caused by corrupted QTI question banks which were fill in the blank questions were exported/imported without any answers. Currently ultra does not allow any question creation without answers.

Content with a cleared release condition cannot be moved out of the Learning module

Under a Learning module, content with release conditions cannot be moved outside the module.

A warning message shows 'This content could not be moved. But after the release condition is cleared, it still couldn't move. Refreshing the browser will allow you to access it.

Individual Feedback Is Not Visible to Students After Grade Override For Group Assessment

When an instructor overrides a student’s grade for a group assignment, the previously provided individual feedback is no longer visible to the student and is replaced with a message indicating no feedback was added. The feedback panel displays the message “Your instructor hasn’t added feedback” instead of the previously entered individual feedback.

Missing Name in From Field for Emails

When the Override "From" Address setting is enabled in Email Configuration, emails sent from a course only display the sender’s email address in the From field. The sender’s display name is not included.

Alt-Text in Blackboard Images Does Not Copy

Randomized Question Pool Tests, When Printed, do not Include all the Questions

Incorrect Grading for Very Small Numbers in Calculated Formula Questions When Using Significant Figures

When using Significant Figures rounding in Calculated Formula questions, answers that are very small numbers (0.0001) are unexpectedly rounded as though it were set to use decimal places rounding instead of significant figures.

The answer is then graded as Incorrect even though the student provided values are mathematically correctly calculated.

Inline Grading Disappears if Formatted Text Containing Named Colors is pasted into a Comment - Copy

When formatted text (Rich Text) containing named color styles such as style="color: red;" is pasted into an Inline Grading comment box, it triggers a JavaScript error. Although the text appears correctly, interacting with the colorized elements causes the entire Inline Grading to disappear.

In some cases, the issue is triggered immediately upon pasting, without any user interaction. This behavior also occurs when copying styled content from word processing applications, such as Microsoft Word or Apple Pages, particularly when the color is defined using a named value instead of a hex code or RGB value.

Annotate Braking/Removing Words Format in Ultra Assignments

When grading a PDF assignment, instructors can add annotations using the Text tool. However, if the selected font size is set to a value smaller than 18, and the page is subsequently reloaded, the text annotations do not persist correctly in the inline viewer. After the reload, the added text may appear broken, incomplete, or partially missing within the PDF displayed in the grading interface. Despite this visual issue, downloading the PDF shows that the annotation text is fully intact in the file, indicating that the problem affects only the rendering or persistence of text annotations in the inline view rather than the saved document itself.

Workaround:

Use a font greater or equal to "18" and also would be to prevent the modification of the font box size.

Text box Cursor Color Does not Change

When instructors use the text annotation tool while reviewing student submissions, changing the text color updates the annotated text but does not update the cursor color to match the selected color.

Large Tests Take Time to Fully Render for Students

Select 'Display one question at a time' or use page breaks to display one page at a time in the test settings to avoid the page loading the entire test.

Late Indicator Not Displaying in Gradebook for Saved Draft Attempts After Due Date

When a student saves an assignment attempt as a draft before the due date, the Gradebook does not display the Late indicator after the due date has passed. The Late indicator displays correctly when there is no submission or when there is a late submission, but draft attempts do not trigger any Late marker. The behavior is inconsistent between instructor view and student view.

Grade Center Displays Incorrect Rounded Percentage for Weighted Total Column

The Grade Center displays an incorrect rounded percentage for the weighted total.

Observed Behavior:

The Weighted Total displays 74.5% even though the rounding and truncation rules indicate that it should display 74.49%

Expected Behavior:

The Weighted Total should be calculated using full precision (up to 15 decimals), resulting in 74.49499999999999%, and should display as:

74.49% when truncated to two decimals

Folders Close After Submitting an Assignment

When an instructor creates a folder in a course and students submit an assignment located within that folder, all folders automatically close once the submission is complete. The student must manually reopen all the folders to navigate to other content. 

Expected Behavior:

Folders should remain open after submitting an assignment so the user can continue navigating through the content without reopening them.

Announcements Not Displaying in the Activity Stream in Ultra

In Ultra view mode, announcements created within a course do not appear in the Activity Stream for enrolled users. Notification settings are correctly configured, but announcements fail to surface after posting.

Observed Behavior:

The newly created announcement does not appear in the student’s Activity Stream.

Expected Behavior:

Posted announcements should appear automatically in the Activity Stream for all enrolled users.

Draft Submissions are Affected by Automatic Zeros and Passing Due Date

Student submission status details are changed after Automatic Zeros has been used for grading on an assignment. If a student saves a draft for an assignment without submitting, the status displays the student draft and that they opened the assessment. Once Automatic Zeros is enabled and the due date has passed the data is removed from the submissions area.

The Status is set to "No Submission" and Student shows "Unopened (late)"

Expected Behavior: 

The status is set to "No Submission" and Student shows "Opened on xx/xx/xxxx xx:xx am/pm (late)"

Flexible Grading Skips Submissions When Using Next Button With Needs Grading Filter

When instructors use the Needs Grading filter in Flexible Grading and navigate with the Next button, not all submissions are loaded if there are many to grade.

  1. Log in as the instructor and go to Gradebook > Overview

  2. Select Grade Now under Needs Grading

  3. Grade the first submission and use the Next button to move through each submission

  4. Continue until the end of the list is reached

  5. Return to the Gradebook Overview page

Observed Behavior:
Not all submissions are loaded through the Needs Grading filter when using the Next button. Some submissions remain in Needs Grading.

Workaround:

Access the “Submissions” tab in the assessment to directly access all submissions

Slow Scrolling When Dragging Content in Ultra

When dragging content items in Ultra courses, page scrolling becomes slow on the course outline page. The issue is most noticeable when moving content beyond the visible screen. A similar slowness, though less pronounced, is also observed when dragging and dropping gradable items in the gradebook.

Feedback Panel Affects the Inline View of Documents on Group Attempts for Assignments Graded with Rubrics

This occurs when opening group attempts for assignments graded with a rubric. If the group attempt is opened while the feedback panel is closed, the uploaded file is rendered inline. Once the feedback panel is opened the frame containing the rendered file is no longer showing.

Gray Screen Displayed When Opening Bb Ultra Exams With Adaptive Release Conditions

When an exam configured with Respondus LockDown Browser includes adaptive release conditions, users may encounter a gray screen with no visible content. However, exams without adaptive release settings function as expected.

Once LockDown Browser launches, the grey screen often occurs. To exit, click the red “x” in the upper right, choose “Close the browser” leave a reason for the exit, and then “save reason and close browser.”

Workaround: Respondus recommends placing the exam inside a folder that has the adaptive release rules applied, rather than assigning them directly to the exam itself.

Release Condition Hides the Student's Grade in the Gradebook

Annotate Becomes Unusable When Browser Zooming

The Annotate feature becomes unusable when the browser zoom level is increased. Specifically, if the zoom is set to 150% or higher, annotation tools may no longer respond or function correctly, preventing users from making annotations.

When the browser zoom is increased to 150% or higher while using the BB Annotate feature:
The screen may turn grey, and clicking elements behind it becomes impossible.
The pencil tool may stop working.
The comments section becomes unresponsive, preventing new comments from being added.
In some cases, the entire page freezes, requiring users to reduce the zoom level back to 100% to regain functionality.

Workaround: Collapse left menu in flexible grading view. Ensure browser is set to 100% zoom and full-screen.

Content Added Outside the RTE in Multiple Choice Questions in an Original Course View are not Preserved After Conversion

The question answers do not contain the content added outside the RTE. It removes Web Links and Images.

In the Ultra experience, the presence of page breaks in an assessment interferes with the Flexible Grading screen's ability to regrade manual questions that have been given full credit. Submissions made after the full credit is granted are not affected.

Workaround: Use Flexible grading to remove the Full Credit option and then use the Content screen to re-add the option.
