How do I fill out the Bottom Line Agreement?

The first person to fill out this form is the student. You will need to read this form in its entirety and then initial by the list of 12 numbered requirements. After that, you will read a small paragraph and sign your full name. After you finish the document, it will get sent to your parent who will sign their name at the bottom. When your parent finishes the document, the completed and signed form will be sent to the UMBC Upward Bound Program. You will also receive a completed copy of the form which you can keep for your records.
