Reasons for Form:
The U-RISE Program Scholarship Agreement is the understanding and acceptance for the Undergraduate Research Initiative for Student Enhancement (U-RISE) Scholarship Support for the 2020-2021 year. It has been given to you as a U-RISE Trainee with the following guidelines. To further understand that being a U-RISE Trainee is a privilege and to promise to strive to work diligently towards pursuing a Ph.D. and a career in biomedical science.
