Curricular Practical Training

This form is for F-1 international students to be eligible for work off-campus and the work experience they intend to pursue is an established part of the curriculum.

The student must complete the form and signature on the bottom to acknowledge that the student is authorized to only work during the specific time period, as well as all the terms in the application. The academic advisor then completes the rest of the form and the Graduate Program Director must sign for approval.
