How do I fill out the Baltimore City Release form?

First the parent names their child’s high school and signs under the paragraph that they are agreeing to. Then, it goes to the student to sign. At this point, the Upward Bound Program will received a copy and request the information from the high school. A copy will be given to you of the document once it is completed, which you can keep for your records.
