What if I have a primary name change or correction, how would I update this information?
Students who have legal documentation of a name change (first, last, and/or middle name) must complete the personal information update form and bring copies of the legal name change document to the Registrar’s Office for verification. Once approved, all student academic records will reflect the new legal name as the primary name or correction.
Examples of Legal Documentation Include: Birth Certificate, Current Driver's License, Court Order, Social Security Card, Marriage License, Divorce Papers, etc.
