The chosen name will not be used on official university documents. The given or primary name will still be displayed on official university documents (i.e. official transcripts, and payroll records).
Please complete the Personal Information Update form in the Registrar's Office, if any your personal information such as primary name and SSN information has changed. Legal documentation will be required. If you are a faculty or staff member, please contact your departmental payroll representative or the Office of Human Resources for assistance.
