How do I fill out the Student Participation Contract?

Upon entering the form, you should read the document in its entirety. After reading the form you will be expected to initial next the the list of 8 numbered requirements and sign your name after the small paragraph at the bottom. Once you have filled out this part, the document will be sent for a parent to sign below where you signed. Once your parent signs you will receive a notification that the document has been completed. You may keep this document for your records. The UMBC Upward Bound Program will also receive a copy of your signed form.
