What is the Baltimore City Release form and why must I fill it out?

This form is filled out as a part of the application to the Upward Bound Program. By signing this form you are authorizing your high school to release your official transcripts, test scores, and any other material the UMBC Upward Bound Program needs to evaluate your application to the program.
