How do I fill out the Parent Participation Contract?

Upon entering the form you should read the document in its entirety. After reading the form, you will be expected to initial next to the list of 5 numbered requirements and sign your name under the 3 paragraphs near the bottom of the page. You will also need to fill out your child’s (the student applying for the program) name. Once you fill out your part, the document will be sent to the second parent to also initial next to the list of 5 numbered requirements and sign their name at the bottom. Once the document is completed by the second parent, you will receive confirmation that the document has been finished. You may keep this document for your records. The UMBC Upward Bound Program will also received a copy of your signed form.
