  • The Graduate Assistant first fills out the form and signs, followed by the Supervisor.

  • On the webform, the Graduate Assistant specifies who the Supervisor is by entering their name and email.

  • When the Graduate Assistant enters the form, they fill out the academic term and year and check the box that applies to them.

  • After that, they sign and it goes to the Supervisor.

  • Once the Supervisor signs, the form is complete.
