SI PASS Leader Contract

Purpose

The Leader Contract is for SI PASS leaders to understand and agree to their responsibilities. It is a contract that verifies the course name and semester taught by the specified leader. Failure to comply with requirements will cause various consequences to occur.

How to fill out the form

1. Verify the Leader name and email are the same as the person starting the form.

2. Use the drop down to select course name and term.

3. Enter the year of the selected term in YYYY format.

4. Click the "Next" button to continue to the Docusign page.

5. Please read through all terms and conditions of the contract.

6. Select the sign button at the bottom of the page to sign the document.

7. Hit "Finish" when you are ready to send the completed form to your supervisor.
