This form is supposed to be filled out when an authenticated member (someone with a myUMBC login) collaborates with an outside (or non-authenticated member).

  • The form is given the ‘Internal’ designation because it is initiated by the Internal member of the collaboration- the UMBC affiliatecontact.
  • The UMBC affiliate contact will have their information automatically placed on the web form, and will be prompted to enter the vendor’s name, email and firm name. The Vendor will then start on the web form, and will be prompted to enter their name, email and firm name.  They will also be asked to enter a UMBC contact name and email, though it is not required
    • From there they are taken to Docu-Sign where they are to select the 'finish'  designation to advance the form to the vendor. 
      • Note: Avoid using the firm's public 'Help Desk' or 'Contact Us' email as they are likely to block DocuSign emails as spam.
    • The Vendor is then sent an email with a link that will take them to Docu-Sign DocuSign to fill out the form, they will need to have various details about their firm including 
      • W-9 Form
      • Maryland Department Of Transportation (MDOT) certification number and type. (If applicable)
      • Small Business Reserve (SBR) certification number and type. (If applicable)
      • List their products or services and whether they are 'green'.
      • List conflicts of interest. 
      The signing process will proceed to the Procurement Department who will be the second and final signature on the form.
