  • This form is supposed to be filled out when an authenticated member (someone with a myUMBC login) collaborates with an outside (or non-authenticated member)
  • The form is given the ‘Internal’ designation because it is initiated by the Internal member of the collaboration- the UMBC affiliate.
  • The UMBC affiliate will have their information automatically placed on the web form, and will be prompted to enter the vendor’s name, email and firm name. 
  • From there they are taken to Docu-Sign where they are to select the 'finish'  designation to advance the form to the vendor. 
  • The Vendor will then start on the web form, and will be prompted to enter their name, email and firm name.  They will also be asked to enter a UMBC contact name and email, though it is not required.
  • The Vendor is then sent an email with a link that will take them to Docu-Sign to fill out the form, they will need to have various details about their firm including 
    • W-9 Form
    • Maryland Department Of Transportation (MDOT) certification number and type (If applicable)
    • Small Business Reserve (SBR) certification number and type. (If applicable)
    • List their products or services and whether they are 'green'
    • List conflicts of interest. 
  • The signing process will proceed to the Procurement Department who will be the second and final signature on the form.
