Second, you will be redirected to the document where you will fill out the required information.

Third, once completed the form will be sent to the Accounting Office to review and sign for approval.

  • Working Fund Custodian Contact Info.

  • Working Fund Custodian Back-Up Contact Info.

How to fill out the form:

The Initiator will begin the form by accessing the web form which they will fill out the info for the specific accounting office information for their specific department. Then you will fill out the remaining information of Cash/Check Transaction Form
