  • This form is supposed to be filled out when a non-authenticated member (someone without a myUMBC login) would like to be an affiliated vendor of the Procurement department at UMBC
  • The vendor has the option to collaborate with UMBC faculty member on this form and list them as a contact, but can also fill it out individually to become affiliated. 
  • The Vendor will start on the web form, and will be prompted to enter their name, email and firm name.  They will also be asked to enter a UMBC contact name and email, though it is not required.
  • The Vendor is then sent an email with a link that will take them to Docu-Sign to fill out the form, they will need to have various details about their firm including 
    • W-9 Form
    • Maryland Department Of Transportation (MDOT) certification number and type (If applicable)
    • Small Business Reserve (SBR) certification number and type. (If applicable)
    • List their products or services and whether they are 'green'
    • List conflicts of interest. 
  • If there is a UMBC contact included in the form, they will receive a CC of the Docu-Sign form, if not the signing process will advance to the Procurement Department who will be the second and final signature on the form.
