This form first goes to the Cardholder to fill out and sign, then the Department Head, then Ann Fusselbaugh.
When the Cardholder first comes to the webform, they will need to fill out their Department Head’s name and email.
When they enter the form, they will need to fill out their department’s name, the last 5 digits of their Visa Card, their telephone number, their default chart string, their building and their room number.
Then, they will check the reasons and give a reason for the maintenance request. If it is for a Cardholder Name Change they will also have to upload documentation showing the name change.
If they are changing, removing, or adding a proxy for the card, they will need to know the name, email, and chartstring for the new proxy.
