- The form is given the ‘Internal’ designation because it is initiated by the Internal member of the collaboration- the UMBC contact.
- The UMBC contact will have their information automatically placed on the web form, and will be prompted to enter the vendor’s name, email and firm name.
- Note: Avoid using the firm's public 'Help Desk' or 'Contact Us' email as they are likely to block DocuSign emails as spam.
- The Vendor is then sent an email with a link that will take them to DocuSign to fill out the form, they will need to have various details about their firm including
- W-9 Form
- Maryland Department Of Transportation (MDOT) certification number and type. (If applicable)
- Small Business Reserve (SBR) certification number and type. (If applicable)
- List their products or services and whether they are 'green'.
- List conflicts of interest.
