  • Purpose
    • When taking photographs or shooting video for stories, promotional materials, or other purposes, it is important to consider whether the subject of the photograph or film needs to complete a consent form prior to publication of their image. Please use this UMBC Photographic, Video, and Audio Consent and Release Form as needed. If you are uncertain if a form is necessary in a particular situation, it is best to err on the side of caution and complete the form.
  • Workflow
    • Initiator → Participant Authenticated/Participant Not-Authenticated → Parent/Guardian (if needed)
  • How to fill out the form
    • First, use the drop-down to select whether the participant is a UMBC student, or a member of the staff or faculty.
      • If the former, enter in the participant’s Campus ID or Employee ID, and their information will automatically be looked up and collected.
      • If the latter, you must manually input the participant’s name and email.
    • If the participant is under the age of eighteen, they will need a parent/guardian to sign: the parent/guardian name and email must also be manually input in order to proceed.
    • Click next to transfer the form to DocuSign, where the names, emails, and other information can be reviewed before sending the form off to be signed by the participant and their parent/guardian.
