  • This form is supposed to be filled out when a non-authenticated member (someone without a myUMBC login) collaborates with UMBC as an institution or a contact that is authenticated with UMBC.The form is given the ‘External’ designation because it is initiated by the Vendor, who is not authenticated.would like to be an affiliated vendor of the Procurement department at UMBC
  • The vendor has the option to collaborate with UMBC faculty member on this form and list a contact, but can also fill it out individually to become affiliated. 
  • The Vendor will start on the web form, and will be prompted to enter their name, email and firm name.  They will also be asked to enter a UMBC contact name and email, though it is not required.
  • From there they are taken The Vendor is then sent an email with a link that will take them to Docu-Sign where they are attaching required legal documents, listing their products or services, address and organization typeto fill out the form, they will need to have various details about their firm including 
    • W-9 Form
    • Maryland Department Of Transportation (MDOT) certification number and type (If applicable)
    • Small Business Reserve (SBR) certification number and type. (If applicable)
    • List their products or services and whether they are 'green'
    • List conflicts of interest. 
  • If there is a UMBC contact included in the form, they will receive a CC of the Docu-Sign form, if not the signing process will advance to the Procurement Department who will be the second and final signature on the form.
